植入年龄对成人语后聋患者人工耳蜗植入术后听觉言语康复效果的影响

摘要

目的探讨植入年龄对双耳重度或极重度感音神经性成人语后聋患者人工耳蜗植入(cochlear implantation,CI)术后听觉言语康复效果的影响。方法回顾性分析10例成人语后聋CI患者(术前听觉剥夺时间均≤1年)的临床资料,根据植入年龄分为成人组(<60岁,5例)和老年组(≥60岁,5例),分别于术后3、6和12个月行听觉行为分级(categories of auditory performance,CAP)及言语可懂分级(speech intelligibility rating,SIR)评估,比较两组结果。结果成人组和老年组患者术后3、6和12个月CAP得分分别为5.20±1.48、7.40±1.34、7.60±1.14分及4.00±0.15、5.80±0.45、6.60±0.89分,10例患者术后SIR得分均为5分,无差异;成人组患者术后6个月CAP得分高于术后3个月,差异有统计学意义(P<0.05),术后6个月与术后12个月CAP得分差异无统计学意义(P>0.05),老年组结果与此一致;成人组与老年组之间术后3、6及12个月CAP得分差异均无统计学意义(P>0.05)。结论 CI可有效改善成人语后聋患者的听觉能力,植入年龄对成人语后聋患者CI术后听觉言语康复效果没有明显影响。
