圆窗入路与鼓阶入路耳蜗开窗人工耳蜗植入术比较

摘要
目的比较经圆窗入路和鼓阶入路耳蜗开窗人工耳蜗电极植入的操作特点及术后康复效果,为合理选择人工耳蜗电极植入入路提供参考。方法回顾性分析103例行人工耳蜗植入术患儿的临床资料,其中53例经圆窗入路、50例经鼓阶入路分别植入人工耳蜗电极,比较两种耳蜗开窗入路的手术耗时、电极植入过程及术后两组患者的有意义听觉整合量表(meaningful auditory integration scale,MAIS)、听觉行为分级(categories of auditory perfomance,CAP)和言语可懂度分级(speech intelligibility rating,SIR)得分。结果两种耳蜗开窗入路人工耳蜗电极均能顺利植入,术中检测电极阻抗均正常,开机后两组对象人工耳蜗均工作正常,听觉反应良好;圆窗及鼓阶耳蜗开窗入路平均手术时间分别为83.4±18.4分钟和102.6±21.5分钟;两组术后MAIS、CAP、SIR得分均较术前提高,组间比较差异无统计学意义(P>0.05),但圆窗入路比鼓阶入路更符合刺激电极植入要求;术后随访24年无相关手术并发症发生。结论人工耳蜗植入术中电极经圆窗入路植入耗时较短,且更安全有效。
