P2Y4受体在面神经损伤后再生过程中的作用

摘要
目的探讨嘌呤能P2Y4受体在面神经损伤后再生过程中对面肌功能恢复的作用。方法 30只健康成年雄性SD大鼠随机分为0.9%生理盐水对照组、P2Y4受体激动剂组和P2Y4受体拮抗剂组,每组10只。在手术显微镜下切断各组大鼠右侧面神经主干后,以Neurolac可吸收共聚高分子外周神经套接管桥接形成神经再生室,同时采用ALZET植入式胶囊渗透压泵分别给予生理盐水(对照组)、P2Y4受体激动剂(激动剂组)和P2Y4受体拮抗剂(拮抗剂组);左侧面神经作为正常对照侧,不做手术处理。术后6周观察各组动物面肌运动恢复情况;采用诱发肌电图检测面肌动作电位的最大幅度、潜伏期;通过面肌Masson染色观察单位面积内的肌纤维数目和平均直径;采用神经干甲苯胺蓝染色观察面神经远心端主干有髓神经纤维的数目。结果各组大鼠术后即出现右侧面瘫,术后6周时面肌功能均有不同程度的恢复,但均无法恢复至正常水平;对照组、激动剂组和拮抗剂组面肌诱发电位最大振幅分别恢复到正常水平的70.6%、84.2%、54.1%;单位面积内面肌纤维数目分别为正常对照侧的72.7%、85.7%、54.7%;面肌纤维平均直径分别为正常对照侧的69.5%、86.0%、和53.3%;单位面积内有髓神经纤维数目分别为正常对照侧的72.9%、83.6%和56.9%;与正常对照侧比较,差异均有统计学意义(均为P<0.05)。但P2Y4受体激动剂组面肌功能及面神经、面肌形态检测指标均较生理盐水对照组恢复更好,而P2Y4受体拮抗组的恢复程度则明显差于生理盐水对照组和P2Y4受体激动剂组,差异均有统计学意义(均为P<0.05)。结论激活嘌呤能P2Y4受体能明显促进受损的面神经再生,从而促进面肌功能恢复。
