4~7岁双耳双模式助听儿童声源定位能力研究

A Study on Sound Localization Ability of 4~7 Years Old Children with Binaural Bimodal Fitting

摘要
目的探讨一侧植入人工耳蜗、另一侧配戴助听器的双耳双模式助听儿童声源定位能力及其影响因素。方法研究对象为51例47岁双耳极重度感音神经性听力损失儿童,平均年龄62.40±7.50月,其中双耳双模式助听组35例,单侧人工耳蜗植入组15例,双侧人工耳蜗植入组1例。采用左右辨别法,以短句为测试材料进行条件游戏测听,测试患儿±90°两个方位的声源定位能力。结果双耳双模式助听组中,9例(25.71%,9/35)有左右声源定位能力,单侧人工耳蜗植入组所有儿童左右声源分辨能力均为猜测水平(无声源定位能力),两组差异有统计学意义(P<0.05)。双耳双模式助听儿童助听器侧补偿效果与声源定位能力呈中度正相关(ρ=0.40,P=0.028),术前助听器配戴时长与声源定位能力呈中度正相关(ρ=0.46,P=0.009)。双侧人工耳蜗植入者将麦克风调整至耳后位置后,其声源定位能力大幅提高。结论双耳双模式助听儿童声源定位能力优于单侧人工耳蜗植入儿童;助听器补偿效果和早期双耳聆听经验可能会影响助听儿童的声源定位能力。
