感音神经性听力损失患者普通话快速噪声下言语测试及评估

The Evaluation of Mandarin Quick Speech-in-Noise Test Materials in Mainland China in Patients with Sensorineural Hearing Loss

摘要
目的探讨感音神经性听力损失患者噪声下的言语识别能力及特点,并制定评价方案。方法选取55例(110耳)感音神经性听力损失患者进行普通话快速噪声下言语测试(Mandarin quick speech-in-noise test,M-Quick SIN),其中男25名,女30名,年龄4192岁,平均67.5±12.1岁;根据气导平均听阈(pure tone average,PTA)分为轻度听力损失组(36耳)、中度听力损失组(59耳)及重度听力损失组(15耳);根据公式"信噪比损失=24.5-答对词数"计算并分析各组信噪比损失(signal-to-noise ratio loss,SNR loss),以此评估其言语感知能力。结果所有患者SNR loss得分范围在-2.592岁,平均67.5±12.1岁;根据气导平均听阈(pure tone average,PTA)分为轻度听力损失组(36耳)、中度听力损失组(59耳)及重度听力损失组(15耳);根据公式"信噪比损失=24.5-答对词数"计算并分析各组信噪比损失(signal-to-noise ratio loss,SNR loss),以此评估其言语感知能力。结果所有患者SNR loss得分范围在-2.521.5dB之间,与PTA无显著相关性;轻度组SNR loss(4.1±4.7dB)明显低于中度组(9.4±6.1dB)及重度组(10.8±6.5dB)(P<0.05);中度组与重度组间SNR loss差异无统计学意义(P>0.05);在轻21.5dB之间,与PTA无显著相关性;轻度组SNR loss(4.1±4.7dB)明显低于中度组(9.4±6.1dB)及重度组(10.8±6.5dB)(P<0.05);中度组与重度组间SNR loss差异无统计学意义(P>0.05);在轻重度感音神经性听力损失时,M-Quick SIN结果可分为轻度信噪比损失(-2重度感音神经性听力损失时,M-Quick SIN结果可分为轻度信噪比损失(-210dB)、中度信噪比损失(1010dB)、中度信噪比损失(1020dB)及重度信噪比损失(≥20dB)。结论本研究初步建立了轻、中、重度感音神经性听力损失者M-Quick SIN的得分评价方案,为临床应用提供了基于M-Quick SIN的听力正常者的鉴别标准,以及感音神经性听力损失患者的分级标准。
