声带息肉患者的主客观嗓音评估特征

摘要

目的分析声带息肉患者主客观嗓音评估特征。方法对66例声带息肉患者(息肉组)及31例健康志愿者(对照组)分别进行动态喉镜、嗓音障碍指数量表(voice handicap index,VHI)、嗓音障碍严重指数(dysphonia severity index,DSI)主客观评估,分析两组的VHI、DSI差异及息肉组VHI与DSI的相关性。结果息肉组66例患者的声带振动幅度、黏膜波、对称性、周期性正常,15例患者声门闭合不全(22.7%,15/66),对照组中2例声门闭合不全(6.5%,2/31);息肉组VHI各值(F、P、E、T)明显高于对照组,差异有统计学意义(t值分别为5.35、9.38、3.42、6.24,P值均<0.01)。息肉组MPT、DSI值明显低于对照组(t值分别为-6.16、-9.27,均P为<0.05);息肉组jitter、F0-High、I-Low值明显低于对照组(t值分别为3.78、3.99、-2.19,P<0.05或P<0.01)。经Spearman等级相关分析,息肉组VHI的T值与DSI无显著相关性(r=0.28,P>0.05)。结论通过动态喉镜、VHI和DSI的综合评估,除部分病例存在声门闭合不全外,声带息肉患者声带振动及黏膜波均正常;VHI评分较正常人明显升高,DSI值明显减小,且VHI的T值与DSI无相关性。
