水平方位声源位置改变诱发健康青年人失匹配负波研究

Measurement of Auditory Discrimination using the Mismatch Negativity(MMN)for the Healthy Youth in the Horizontal Plane

摘要
目的探讨水平方位声源位置改变能否诱发失匹配负波(mismatch negativity,MMN),并定量研究能够诱发出失匹配负波的两个声源间的最小角度,获得一种客观、简便的测试声源定位能力的检测方法。方法在无回声自由声场环境下,利用实验室自主研发的声源定位测试仪(国家专利号200520017252.1)和Bio-logic听觉诱发电位仪,采用oddball听刺激序列和被动听觉模式,以1 000Hz纯音作为刺激声,声源位置的变化作为刺激诱发模式,对30例健康青年受试者按照角度辨别阈法的测试原理引出MMN,记录受试者水平0°、±45°、±90°五个方位的角度辨别阈值(minimum audible angle,MAA)和对应的MMN潜伏期和波幅。结果所有受试者均能成功引出MMN波,不同方位的MAA分别为:MAA(0°)=2.09±1.81°,MAA(-45°)=3.84±1.61°,MAA(45°)=3.69±2.39°,MAA(-90°)=4.41±1.41°,MAA(90°)=4.23±3.22°。其中正前方0°角度辨别阈值最小,与其他方位角度辨别阈值差异有统计学意义。在水平左右对称位置(即-45°与+45°,-90°与+90°),MMN潜伏期、波幅差异无统计学意义。MMN潜伏期随着声源偏侧化程度的增加而延长,组间差异有统计学意义(P<0.05),±90°方位诱发MMN波幅较0°和±45°方位处增大,潜伏期延长,差异有统计学意义(P<0.05)。结论水平方位声源位置改变可成功诱发失匹配负波,采用失匹配负波研究声源角度辨别阈值,可作为一种客观、简便、定量评估声源定位能力的方法。
