青少年喉鳞状细胞癌临床特点分析

摘要

目的探讨青少年喉鳞状细胞癌的临床特点。方法回顾性分析一例青少年喉鳞状细胞癌患者的临床资料,并结合文献复习,与成人喉鳞状细胞癌进行对比。结果患儿,女,1996年12月出生,自6岁开始无明显诱因出现渐进性声嘶,14岁时(2010年初)声嘶加重,纤维喉镜检查见右侧声带呈淡红色结节状突起,累及部分室带及声门下,右喉室消失,右侧声带固定于近中线位;病理诊断为喉鳞状细胞癌,颈部CT及MRI检查见右侧声门及声门上、下区肿瘤,临床诊断为喉鳞状细胞癌(T4N0M0),行放化疗后一年肿瘤复发,行喉全切除术,术后一年(2012年3月)患儿出现多脏器肿瘤转移,器官功能衰竭死亡。复习文献,青少年喉鳞状细胞癌起病隐匿,易发生远处转移,病因可能与HPV感染所致的喉乳头状瘤有关,其治疗目前遵循成人方法,但对保留喉功能的要求更高。结论青少年喉鳞状细胞癌起病年龄小,临床症状不典型,对有喉乳状瘤病史及持续或渐进性声嘶的患者,应尽早行喉镜检查,必要时取活检,以免误诊。
