新生儿听力筛查7064例分析

Analysis of 7064 Cases of Neonatal Hearing Screening

摘要

目的分析新生儿听力筛查结果 ,为先天性听力损失患儿早期诊断和干预提供依据。方法对2004年1月～2008年12月在上海市东方医院产科出生的7064例活产新生儿应用畸变产物耳声发射进行新生儿听力筛查,初筛未通过者于生后42天复筛,复筛仍未通过者于3～6月龄时通过声导抗、ABR等检查进行听力学诊断。结果 7064例中正常新生儿6412例,初筛未通过579例(9.03%,579/6412),复筛513例,复筛未通过38例(7.41%,38/513);重症监护儿652例,初筛未通过129例(19.79%,129/652),复筛107例,复筛未通过20例(18.69%,20/107)。正常新生儿与重症监护儿听力筛查初筛与复筛未通过率比较差异有显著统计学意义(P<0.01)。复筛未通过的58例中,46例于3个月后行诊断性检查,最后确诊为先天性听力损失17例(26耳),检出率0.24%(17/7064),其中,轻度听力损失5例(6耳),中度8例(13耳),重度4例(7耳),正常新生儿8例(12耳),检出率0.13%(8/6412),重症监护儿9例(14耳),检出率1.38%(9/652)。结论新生儿听力筛查可及早发现先天性听力损失患儿,重症监护儿是听力损伤的高危人群,其跟踪随访尤为重要。
