非典型症状听神经瘤的临床分析

摘要

目的了解非典型症状听神经瘤患者的临床特点,为其诊断提供参考。方法回顾性分析2004年1月～2006年12月诊治的137例随访资料完整的听神经瘤患者的临床资料,比较有非典型症状与无非典型症状听神经瘤患者的年龄、病程、听神经瘤在MRI中最大直径及听神经瘤生长指数的差异等。结果 137例中27例(非典型症状组,19.70%)有非典型症状,其中,11例(40.74%,11/27)突发性聋,5例(18.52%,5/27)头胀痛,3例(11.11%,3/27)中耳炎,4例(14.81%,4/27)耳闷胀感,耳痛、发作性眩晕、味觉异常及枕区疼痛各1例(各3.70%,1/27)。与无非典型症状听神经瘤患者(典型症状组,110例)比较,非典型症状组患者的病程较典型症状组短(P<0.05),肿瘤生长指数大于典型症状组(P<0.05);年龄及MRI肿瘤最大直径两组间无统计学差异(P>0.05)。结论听神经瘤的非典型症状主要表现为突发性聋、头胀痛、耳闷胀感等,可能与肿瘤生长速度较快有关;对具有非典型症状的患者,也应警惕听神经瘤的可能。
