前庭神经炎52例临床分析

摘要

目的分析前庭神经炎(vestibular neuritis,VN)的临床分类、表现及前庭上、下神经的受累情况。方法分析52例VN患者的临床表现及病程经过,并行前庭双温试验和前庭诱发的肌源性电位(vestibular evoked myogenic potential,VEMP)检查。结果52例VN患者中,单发性49例,复发性3例;单侧50例,双侧2例;除2例患者表现为平衡障碍外,其余均表现为旋转性眩晕;前庭双温实验和VEMP均异常(即前庭上、下神经均受累)者20例(38.5%,20/52),仅VEMP异常者(仅前庭下神经受累)7例(13.5%,7/52),仅前庭双温实验异常(仅前庭上神经受累)25例(48.0%,25/52);仅VEMP异常者主要表现为平衡障碍,前庭双温试验和VEMP均异常者则主要表现为旋转性眩晕,前庭上神经受累的几率明显高于前庭下神经,差异有统计学意义(P<0.01)。结论VN复发性少见,且多数患者没有病毒感染史;主要累及单侧前庭上神经或上、下神经,而单独累及前庭下神经者少见;前者表现为眩晕和(或)平衡障碍,后者主要表现为平衡障碍。
