摘要
目的 探讨畸变产物耳声发射 (distortionproductsotoacousticemmission ,DPOAE)与超高频纯音听阈用于早期耳蜗性听力损害的听力监测的可能性。方法 DPOAE以配对初始音诱发 ,两音强度均为 70dBSPL ,频率比为1∶1 2 ,频率范围 0 5~ 8kHz。测定常规频率 (12 5Hz~ 8kHz)及超高频率 (9~ 2 0kHz)的纯音听阈。比较正常听阈组与不正常听阈组的DPOAE振幅的改变 ,对DPOAE幅值与纯音听阈进行相关分析和直线回归分析。结果 受试者中 ,近半数耳存在超高频听阈升高。不正常听阈组DPOAE的高频段 (3~ 6kHz)幅值显著低于正常听阈组 (P <0 .0 1)。DPOAE高频段平均幅值与 11 2、12 5、14、16、18kHz的超高频听阈直线相关 (r =0 .6 0 1,P <0 .0 1)。结论 DPOAE的幅值与超高频纯音听阈具有一定的相关性 ,两者均可能用于监测早期的耳蜗性听力损害
