噪声下普通话数字串听力筛查测试材料的开发与验证
Development and Validation of Digital Triplet Test Materials for Mandarin in Noise

【Abstract] Objective To develop and apply the Mandarin version of the broadband digit triplet test ( BB- DTT) and the low-pass digit triplet test (LP-DTT) to explore their sensitivity and specificity in detecting early high frequency hearing loss, with a view to providing a rapid and effective screening tool for clinical use. Methods The guide word “number” and digit 0~9 were recorded, each number was adjusted to the same RMS loudness level and then connected with the guide word to form a digital triplet test material. The broadband noise and low pass filter noise of the test material were formed by repeated stacking of digital strings. A total of 40 adult subjects were recruited, including 20 normal hearing cases and 20 high-frequency hearing loss cases. Pure tone audiometry, acoustic immittance, BB-DTT and LP-DTT test were performed successively. The homogeneity of the test material was ensured by the results of each digit at the front, center and rear positions. The validity of the test material was analysed by the correlation,eigenvalue, sensitivity and specificity of the test results and the average hearing threshold of high frequency. Results ①The SRT threshold of BB-DTT and LP-DTT was highly correlated with the high frequency average hearing threshold of pure tone (Pearson correlation coefficient was 0.73 and 0.71 respectively, P<0. 01). ② The optimal cut off point of BB-DTT and LP-DTT test were-10.5 dB and -15.2 dB respectively. The area under the subject operating characteristiccurve (AUC) of BB-DTT and LP-DTT were 0.97 and 0. 96 respectively.③ The sensitivity and specificity of BB-DTT were 0.90 and 0.95 respectively. The sensitivity and specificity of LP-DTT test materials were 0. 95 and 0.85 respectively. Conclusion The Mandarin version of BB-DTT and LP-DTT developed in this study has high sensitivity and specificity in detecting high frequency hearing loss, and can quickly and effectively screen early hearing loss.
