助听器对老年听障者认知功能影响的Meta分析

Meta-analysis of Cognitive Effects of Hearing Aids on Elderly Hearing-impaired Patients

摘要
目的 系统评价助听器对老年听障患者认知功能的影响,为老年性聋的临床干预提供参考。方法 在PubMed, Embase, Cochrane, Web of Science四个外文数据库中检索助听器对老年听障患者认知功能影响的临床研究,检索时间均为建库至2022年9月。按照Cochrane协作网评议员手册评价纳入文献的质量,采用纽卡斯尔-渥太华量表(NOS)评价研究偏移风险,利用Revman 5.4软件进行Meta分析,并进行亚组分析确定异质性来源。结果 共纳入17篇符合条件的外文文献,包括1 107例患者。Meta分析结果显示,助听器干预者的认知得分优于未干预者[SMD=0.28,95%CI(0.09,0.47),P<0.05];助听器干预后的认知得分优于干预开始时[SMD=0.58,95%CI(0.22,0.95),P<0.05];亚组分析结果显示已存在认知障碍的老年听障患者助听器干预前后认知得分无显著变化[SMD=0.11,95%CI(-0.30,0.51),P>0.05];漏斗图结果显示无发表偏移。结论 助听器干预有利于改善一般老年听障患者的认知功能,对已存在认知障碍的老年听障者无显著影响,但结论仍需进一步证实。
