多元复合声治疗耳鸣的短期疗效及其对额极皮层氧合血红蛋白浓度的影响

摘要
目的 研究慢性主观性耳鸣患者额极皮层(frontal polar cortex, FPC)血流动力学特征,探讨多元复合声(multielement integration sound, MIS)治疗的短期疗效及其对FPC血流动力学指标氧合血红蛋白(oxyhemoglobin, HbO)浓度的影响。方法 以慢性主观性耳鸣患者50例(耳鸣组)及年龄性别相匹配的无耳鸣者50例(对照组)为研究对象,分别接受15 min的MIS治疗。耳鸣组治疗前后进行耳鸣障碍量表(tinnitus handicap inventory, THI)、视觉模拟评分(visual analogue scale, VAS)评估;两组患者治疗前后均用功能性近红外光谱技术(functional near infrared spectroscopy, fNIRS)测试额极皮层8 min的平均HbO浓度,比较两组治疗前后HbO浓度变化,分析临床特征与HbO的相关性。结果 耳鸣组治疗前FPC的HbO浓度较对照组高,治疗后耳鸣组FPC的HbO浓度降低,VAS得分降低;对照组MIS治疗后FPC的HbO浓度增加。耳鸣组MIS治疗前后HbO浓度的差值(ΔHbO)与治疗前后VAS得分差值(ΔVAS)正相关。结论 慢性主观性耳鸣患者额极皮层血流动力学异于无耳鸣对照组,MIS治疗可改变其额极皮层的血流动力学,额极皮层可能是MIS的作用部位。
