离体喉模型中声带截短术对声学及空气动力学影响的初步探索

Exploration of the effects of wendler glottoplasty on acoustics and aerodynamics in an ex vivo experiment

摘要
目的 通过离体喉发声模型探索声带截短不同长度对音调、发声能力及发声质量的影响。方法 利用30只离体狗喉建立发声模型,对照组及实验组各15只,实验组离体喉先后在声带截短1/4及1/3状态下行声学及空气动力学测量,对照组不作处理进行测量。采集指标包括基频(F_0)、振幅微扰(shimmer)、基频微扰(jitter)、谐噪比(harmonic-to-noise ratio, HNR)和声强等声学指标及发声阈压(phonation threshold pressure, PTP)、发声不稳定压力(phonation instability pressure, PIP)、平均流量(mean flow rate, MFR)、发声阈流(phonation threshold flow, PTF)和发声不稳定流量(phonation instability flow, PIF)等空气动力学指标。结果 与对照组相比,1/4声带截短组的F_0平均提高了80 Hz,但jitter、PTF显著变差(P<0.05或P<0.001);1/3声带截短组F_0提高了132 Hz,同时shimmer、jitter、PTF和PTP均显著变差(P<0.05或P<0.001)。与1/4声带截短组相比,1/3声带截短组的F_0、jitter、PTP和PTF均显著升高(P<0.05或P<0.001)。结论 随着声带截短长度的增加,基频显著提高同时声学及空气动力学指标恶化。声带截短术后音调的提高会以减小发声能力及降低发声质量为代价。
