离地性眼震水平半规管BPPV诊治策略

Strategy in the Diagnosis and Treatment of Horizontal Semicircular Canal BPPV with Apogeotropic Nystagmus

摘要

目的探讨离地性眼震水平半规管良性阵发性位置性眩晕(HC-BPPV)的诊治策略。方法回顾性分析2017年7月～2019年6月确诊的48例离地性眼震HC-BPPV患者的临床资料,均采用滚转试验(roll test)、低头仰头试验(bow and lean test)等方法判断侧别,通过Gufoni法、Barbecue法、Head shaking法(向左右快速甩头)以及强迫侧卧体位法(FPP)法多方法运用判断耳石位置以及手法复位,观察治疗效果。结果 48例患者床边手法复位治疗,首诊时31例经Gufoni法、Head shaking法转为向地性眼震,予以Barbecue翻滚法复位,30例症状缓解,1例减轻;17例未转为向地性眼震,予以前庭抑制剂、Barbecue翻滚法及FPP法治疗。第3天复诊时,转为向地性眼震的31例患者中,30例无眩晕及眼震,仅1例仍有两侧向地性眼震,Barbecue翻滚法复位后眩晕及眼震消失;另17例未转为向地性眼震患者中,10例转为向地性眼震者再行Barbecue翻滚法复位症状缓解,1例Gufoni法复位眩晕及眼震即刻消失,1例Gufoni法复位后仍存在两侧离地性眼震,予以Head shaking法习服及FPP法,5例未诱发出眩晕及眼震。1周后随访时,所有患者眩晕消失,残留程度不一的头昏症状。结论离地性眼震HC-BPPV的诊治,最重要的是通过眼震变换明确耳石位置,多方法运用可提高即刻复位效果,缩短病程。
