人工耳蜗植入前后耳鸣与焦虑、抑郁相关性分析

摘要

目的探讨人工耳蜗植入前后患者耳鸣与焦虑、抑郁相关性及其影响因素。方法选择2019年10月至2020年12月于安徽医科大学第一附属医院行人工耳蜗植入术且术前伴有耳鸣的成年患者39例，所有患者均在手术前一天及术后6个月独立自主完成耳鸣致残量表(THI)、汉密尔顿焦虑量表(HAMA)、汉密尔顿抑郁虑量表(HAMD)问卷调查，观察患者人工耳鸣植入前后耳鸣变化、焦虑及抑郁随耳鸣程度不同的变化特点，分析术前术后耳鸣变化与焦虑、抑郁变化的相关性及影响因素。结果术后92.31%(36/39)患者耳鸣症状有所改善，术后THI评分较术前明显降低(P<0.001);人工耳蜗植入前及植入后随着耳鸣严重程度加重，焦虑及抑郁程度均加重；人工耳蜗植入前后耳鸣严重程度改变与焦虑(r=0.925,P<0.001)、抑郁(r=0.887,P<0.001)状态变化均为正相关；在THI的功能、情绪和灾难性三个维度中，情绪性改变与焦虑(r=0.880,P<0.001)、抑郁(r=0.807,P<0.001)程度改变的相关性最强且为正相关；焦虑程度中精神性因子的改变(r=0.862,P<0.001)、抑郁程度中焦虑/躯体化因子改变(r=0.839,P<0.001)与耳鸣严重程度改变的相关性最强，且均为正相关。人工耳蜗植入前后耳鸣严重程度改变与抑郁中认知障碍、日夜变化、阻滞及绝望感四种因子变化无明显相关性(P>0.05)。结论对于术前伴有耳鸣的人工耳蜗植入者，其耳鸣与焦虑、抑郁情绪密切相关，在临床工作中应适当关注此类耳鸣患者的情绪问题。
